You must show evidence that you have no money and no assets. Google "judgment proof letter" and check out the debt-and-credit letters website. There is a sample letter and a template you can use to create your own letter that includes all the necessary statements.

Just state that you have been declared so and includes the basics, the jugde who signed off, the state's law and the circumstances why you have this. Send a copy.

The best way is to lay out for financial position, show all your income or lack of and a list of all your debts and judgments

There are free forms online that can guide you. You need to clearly state that you have received a demand on your income, state date, amount etc, clearly state your wish to pay, and your inability to do so. I strongly suggest you find a form online and follow it.
