Be able to state what you are giving up or have actively done to prevent a re-occurrence. For example, if you cheated with someone from work, quit the job as a sign of good faith. If it was from someone met at a party, never go to a party again without your girlfriend.

Be honest with her. Apologize and don't make excuses. You'll need to re-gain her trust. Ask her if you can start over and take her on a 'first date'.

This is very difficult, but not impossible. You'll have to sincerely convince her this will not happen again, and demonstrate how you have changed. Let some time pass, then talk.

Start by giving the issue time to breath. Eventually, call to set up a time to meet in person. When you meet, show her she can trust you.
