You need to know what state and county you were born in. You need to contact the records department or vital statistics department and they will tell you their procedures.

Since 9/11 it is no longer a case of just requesting one. You have to prove your identity some other way. You will have to have a valid driver's license, passport and social security. You will also need to prove residency with a bill and sign an affadavit saying you are that person.

This depends on where you were born. If in the United States it would be through the state's and county's Department of Heath. They usually have an office of birth and death records or vital records were you can request a new certificate.

You can make a request to the agencies in the state where you were born. There is a fee. Search for "find birth certificate." You can then contact the agency for information about the fees and how to make the request. Online services are available, but often cost more, so be careful.
