Does a tune-up always involve getting a new fuel filter?

I have a 2003 Dodge Caravan.

Depends on the mileage and if you are experiencing lag, (hesitation when the pedal is depressed). Generally my experience on my own vehicle is to replace the fuel filter every 100,000 to 150,000 miles.

IT depends on how often you get a tune up. In most vehicles the fuel filter is only changed every 30000 miles. if you get your car tuned up before this mark then you don't need to replace it. IF you wait for every 30000 miles then yes it would.

You don't HAVE to get a new fuel filter each time you get a tune up. However, it is RECOMMENDED that you do so. If the vehicle is in need of a tune up then it's likely that is one of the parts which needs to be replaced in it.

Unless you ask to have it replaced a shop probably won't replace it unless there is an issue. Typically there is a maintenance guide for the vehicle that list at what time the filter should be inspected or replaced.
