Yes. In most cases if only one parent is taking the child you have to have written consent from the other parent. You also need their birth certificate.

New security guidelines stipulate that all individuals traveling by air need a passport. Children under 16 can still cross sea and land borders with a certified US birth certificate.

A child needs a passport to any place that an adult needs a passport to enter. Passports are available to a child as young as newborn, and just needs the parents signature in place of the child. Children passport expire the same as a regular passport and the photo's can be taken at any passport photo center.

Yes, everyone needs a passport to travel to the Dominican Republic, children under 18 years of age without one or both parents also need a notarized document from the parent(s).
