You can either talk to your Human Resources office to see if you can take this time off, or get a note from your mental health provider.

You can get time off for work for depression but you have to get a doctor to sign a FLMA (family medical leave act) form saying that your are clinically depressed. You also can't come back to work until the doctor oks it.

Probably. You will need to speak to your human resources team and you may need a referral from a medical professional.

Most employers do not accept depression as a valid illness. If they do you will probably need to have paperwork stating you are on medication for the condition from a psychiatrist. But as a general rule, any time off for depression is considered personal time.
