This generally depends on the organization with which you are required to have a password for and the type of information you will be guarding. Financial and employment sites usually require these, but other sites like Yahoo and Google often let you make your own.

It depends on the website. Some sites allow numbers in the screename, but not symbols. Most will allow just upper and lower case letters. If they allow numbers, they'll often suggest usernames that are similar, but with different numbers after the name. Some allow spaces and some only allow the _ between words.
