Are the runts of the litter the most sickly of the puppys?

Small size can be due to a birth defect or health problem or from the placenta being smallest due to crowding in the uterus. Small does not always equal sickly.

A runt can be sickly so get it checked out but it does not mean the pup is sick for sure. Many are just smaller and don't right for food.

Not Sickly no. Runts are the smallest of the litter and have a hard time getting enough to eat. I have 1 Runt who is Weighs about 3lbs. But she is a grown kitten. So you should be fine.
